Illustration of Oiwa, a Japanese Onryō (怨霊) or malevolent spirit from the very well known and popular ghost story 四谷怪談 (Yotsuya Kaidan), my absolute favourite ghost tale in Japanese folklore, which is said to be partially based on real life events.

Yotsuya Kaidan is the tragic tale of Oiwa, whose husband's betrayal in life transforms her into a vengeful spirit once she encounters death. Her most alluring and creepy characteristic that sets her apart from other ghosts of this kind is the malformation on her left eye, caused by the poison she is given by her husband and the family of his younger suitress. Also, there is an accidental stab at her neck, but I believe this is a detail that has been omitted in most adaptations of the tale.
