In the summer of 2013, I worked as a graphic design intern at a dental operations center. I did a lot of paperwork design for them, including this referral slip, which was distributed to the dental offices to fill out for patients. My boss gave me the basic information and we collaborated on how to display and edit the information to fit the referal slip cohesively and attractively. This is the finished product, front and back, for the design, which was printed in the form of pads, so they could be filled out and ripped off the pad to hand to individual patients.
