Landing page shows the assessments that are completed by the User. A filter option is used to switch among Completed/In Progress assessments. User can add a new assessment from these pages.

In Progress Assessments

Assessment Page. The list of assessments page is not shown in this demo. From the list user can select an assessment and then complete the questionnaire associated with it. Rating is provided for each question.

Submit assessement page - Based on individual ratings, the site is provided with an overall rating. if the rating is Medium/Poor user is provided a recommendation field.
