It is a Valentine gift made for Ray, 2015.

The whole set contains three coupons,
a card with instruction & signature,
and an envelope with a sticker.

I was inspired by the arcade game machine coupons.
Here are the three different 'services' shown on the coupons:

3 the date was printed by self-inking kit
4 card with insturction and signature at the back,
also a cut to insert the coupons together.
