Since 2018, Wayne Macfadden, MD, has provided Psychiatric evaluations and treatment for Native Americans with Alcohol Use Disorder (AUD) at the Spirit Lake Reservation in North Dakota.

AUD affects more than 15 million people in the United States. AUD is differentiated from non-pathologized moderate alcohol use based on the following factors:

1. Craving - In addiction treatment, craving refers to a compulsive need to consume alcohol. Cravings are usually triggered by specific environmental or emotional situations.

2. Withdrawal - Due to physiological and psychological dependence on alcohol, people with AUD may experience severe symptoms if they are not able to access alcohol. This includes physical symptoms such as autonomic hyperactivity, sometimes leading to physically shaking, and even seizures..

3. Lack of Control - People with AUD may feel compelled to drink more alcohol or drink more frequently despite experiencing negative consequences due to drinking. It can also be difficult for people with this disorder to stop after one or two drinks, thus losing control.
